RVU Calculator

Written by Blake Boege

An RVU (Relative Value Unit) calculator is a medical billing and productivity utility designed to calculate Medicare reimbursement and physician compensation. It aggregates Relative Value Units for physician work (wRVU), practice expense (peRVU), and malpractice (mpRVU), and optionally applies Geographic Practice Cost Indices (GPCIs) before multiplying the total by the annual CMS Conversion Factor to estimate payments.

Estimate physician compensation and Medicare billing reimbursement. Enter the physician work (wRVU), practice expense (peRVU), and malpractice (mpRVU) components, customize Conversion Factors, and optionally apply geographical GPCI adjustments.

Quick Answer

Calculate medical Relative Value Units (RVUs) and estimate Medicare payments. Enter wRVU, peRVU, and mpRVU values to view total adjusted RVUs.

Relative value of physician time and skill.

Relative value of office rent, equipment, and staff.

Relative value of liability insurance costs.

Annual rate translating RVUs to dollars (2025 CMS CF is $32.35).

Examples

2.5 wRVU, 1.5 peRVU, 0.2 mpRVU (Unadjusted, $32.35 CF)

Total RVUs: 4.20, Reimbursement: $135.87

1.5 wRVU, 1.0 GPCI work, 1.1 GPCI PE, $32.35 CF

Adjusted RVUs: 2.80, Reimbursement: $90.58

How it works

Relative Value Units are multiplied by a conversion factor to determine reimbursement. When GPCI indexes are provided, they adjust each individual component before calculating the final total:

Unadjusted RVU Formula:

Total RVUs = Work RVU + PE RVU + MP RVU

Geographically Adjusted RVU Formula:

Adjusted RVUs = (Work RVU × Work GPCI) + (PE RVU × PE GPCI) + (MP RVU × MP GPCI)

Estimated Reimbursement:

Reimbursement = Total RVUs × Conversion Factor

Frequently asked questions

A Relative Value Unit (RVU) is a standard measure used by Medicare and other insurers to determine the reimbursement value of medical services. It is based on three components: physician work (wRVU), practice expense (peRVU), and malpractice expense (mpRVU).

The Conversion Factor (CF) is a dollar amount determined annually by Congress and CMS that translates total Relative Value Units into an actual monetary payment amount. For example, in 2025, the standard CMS conversion factor is set at $32.35.

Geographic Practice Cost Indices (GPCIs) are cost modifiers applied to each of the three RVU components to account for varying costs of living, practice operation, and malpractice insurance in different regions across the United States.

Physicians, practice managers, and healthcare administrators use it to track doctor productivity (often measured in wRVUs), evaluate contract compensation, and estimate insurance reimbursement rates for specific CPT codes.
